What a Fat Dissolving Injection Is Actually Doing Under Your Skin

A fat dissolving injection — clinicians call it injection lipolysis — places a small volume of solution into the fat layer under the skin. Its best-studied ingredient is deoxycholic acid, a cytolytic agent: it breaks open the outer wall of the fat cell so the cell cannot survive.

Why the swelling is the treatment, not a side effect

Dermatology textbooks describe injection lipolysis as producing a temporary panniculitis, a deliberate and short-lived inflammation of the fat layer, which is why swelling over the first 2 to 3 days is part of how the treatment works rather than a sign that something has gone wrong.

Panniculitis simply means inflammation in the fatty layer under the skin. Injection lipolysis works by dissolving the membrane of the fat cell itself, which is why the treated area feels inflamed and tender for the first 3 days.

Why the change shows up over weeks, not days

Nothing disappears the moment the needle leaves. Broken-down fat has to be carried away and processed. That is why published work measures results months after the last session, and why one appointment rarely delivers the contour you pictured.

Who needs to take aftercare most seriously

Some people should be stricter. Bruising is more likely on aspirin, other anti-inflammatory painkillers or a blood thinner, and healing is slower if you smoke. Marks left by inflammation fade slowly on deeper skin tones. If a previous injectable left lumps, say so beforehand.

One word deserves care. The warmth is local: the area feels hot to your fingertips, the way a bruise does. That is not the same as running a temperature. A whole-body fever is not normal here.

Day 2 to day 3: normal reactions start to settle

Itching, warmth, tenderness, redness and swelling typically last about 3 days before they start to fade. Most patients find day two the least comfortable and day three noticeably better. Minimal swelling does not mean the injection failed.

What not to do in the first 24 hours

Four things cause most avoidable problems: rubbing the area unless told to, hot showers and saunas, alcohol and cigarettes, and hard workouts. Each is explained below.

After day three the story shifts from inflammation to resolution. Color, texture and sensation each run on their own timetable.

Bruising at the injection sites generally clears within 1 week, though this varies from person to person. It is one of the most commonly reported reactions to injections of any kind, so prevention is the most useful part of fat dissolving injection aftercare.

Firm spots and small lumps you can feel

Firmness — clinicians call it induration — is on the expected list of injection-site reactions. You may feel a hardened patch or a few pea-sized spots, more obvious to your fingers than to anyone’s eyes. In injectable treatments generally, careful placement and gentle massage prevent most of them.

Reduced sensation is on the same list and is typically temporary. Most people describe the skin as muffled rather than dead, and it fades as the swelling does. Numbness that is spreading rather than shrinking deserves a phone call.

How to Prevent Bruising Before You Even Sit Down

Most aftercare advice starts when the appointment ends. That skips the biggest lever you have over bruising: the week and a half before.

What to pause for about 10 days before

Cosmetic dermatology literature on injectable treatments advises pausing aspirin, other NSAIDs, green tea and vitamin E for about 10 days before an appointment, because each of them makes injection-site bruising more likely.

• Aspirin, and painkillers such as ibuprofen and naproxen
• Vitamin E supplements
• Green tea in large or concentrated amounts
• Anything else your doctor has flagged as thinning the blood

One caveat matters most: this is not permission to stop a prescribed medication. If you take a blood thinner or daily aspirin on medical advice, keep taking it and tell us instead. The same applies to aftercare after a fat grafting procedure.

Cold packs before and right after

An ice pack applied immediately before an injection has been reported to reduce both pain and bruising. A cold pack over a cloth afterwards is harmless — short spells, and never ice directly on skin.

Reach for acetaminophen, not ibuprofen, afterwards

For injectable treatments in general, acetaminophen (sold as paracetamol or Tylenol) is preferred over anti-inflammatory painkillers for tenderness afterwards, precisely because NSAIDs can make bruising worse. Most patients need nothing at all.

What to Avoid After Fat Dissolving Injections — and Why

Alcohol on the day of your treatment

Smoking and nicotine

Research on nutrition and wound healing in aesthetic surgery, published in 2024, describes 3 separate problems caused by smoking: nicotine narrows small blood vessels, carbon monoxide binds to hemoglobin and limits oxygen transport, and neutrophil activity falls during the inflammatory phase.

Saunas, hot spas and heat for about one week

Saunas and hot spas should be avoided for about 1 week after a fat dissolving injection. Heat widens blood vessels, and widened vessels in already inflamed tissue mean more swelling and bruises that spread before they fade. Jjimjilbang visits and hot yoga count too.

Water and gentle movement

Drinking more water than usual is worth keeping, though not for the reason the internet gives. Water does not flush out toxins. It supports the circulation and lymphatic drainage that carry away what the injection broke down. Walking helps the same system without adding heat.

Skin Care After the Injection: Barrier Cream, Ointment and Sunscreen

Three products cover the surface while the work happens underneath — the easiest part of fat dissolving injection aftercare to get right: a barrier repair cream, the antibiotic ointment you were given, and sunscreen.

The first day around the needle marks

Apply the ointment as directed to the injection points and keep the surrounding skin moisturized. Skin with a weakened barrier is more prone to irritation and contact dermatitis, and barrier repair moisturizers reduce how often that happens.

Sunscreen and pigmentation

Guidance for treating richly pigmented skin recommends a sunscreen of at least SPF 30 together with hats and umbrellas, because studies show that ultraviolet exposure makes post-inflammatory pigmentation darker and slower to fade.

A sunscreen of at least SPF 30 is advised on any treated area that sees daylight. This matters most if your skin tone is deeper, where marks left by inflammation are more likely and more stubborn. Shade and a hat help as much as the bottle.

Sessions, Intervals and Combining With Radiofrequency

One appointment is a start, not a course. Two questions come up constantly: how many sessions, and whether to add radiofrequency.

Why one session is usually not enough

It is not a way to melt fat faster: it targets the collagen-rich tissue beneath the skin, and the effect is tightening. The trend is lower-energy treatment over several passes, and results depend on the operator’s technique.
